Hours of Work: Part-time, 22.5 hours/week.
What you’ll be doing:
- Verify Computerized Provider Order Entry (CPOE) in BDM system by monitoring drug therapies, detecting therapeutic incompatibilities, identifying patient’s medication-related problems, reporting clinical warnings and advising interventions to physicians and other healthcare practitioners
- Create and maintain computerized patient medication profiles
- Monitor patient medication regiments and lab results related to drug therapy
- Collect Best Possible Medication History on patient’s admission
- Perform Medication Reconciliation across transitions of care
- Check for accuracy and completeness of all products distributed from the pharmacy
- Research and provide pharmaceutical information to all healthcare providers as requested
- Participate in multidisciplinary team meetings
- Conduct patient medication teaching groups; arrange interviews with individual patients and families as requested
What we’re looking for:
- Bachelor of Science in Pharmacy
- Current registration with the Ontario College of Pharmacists required - Part A
- Experience with Electronic Medication Administration Record (EMAR) and CPOE an asset
- Unit Dose System experience preferred
- Hospital residency program or equivalent an asset
- Highly developed communication and interpersonal skills
- Excellent problem solving and conflict management skills
- Computer literacy
- Strong organizational and multitasking skills
- Strong attention to detail
- Proven ability to work independently and with a team
- Previous experience in a mental health facility is an asset
