SickKids is one of the world's foremost paediatric health-care institutions and Canada's leading centre dedicated to advancing children's health. Nurse Practitioners are integral to this mission. We are seeking a Nurse Practitioner to provide advanced clinical leadership within the Infectious Diseases Program, with a focus on special populations.

The Nurse Practitioner will promote interprofessional excellence and collaboration in child- and family-centred care through five core role components: expert clinical practice, education, consultation, research, and leadership/change agency.

Here's What You'll Get to Do

Provide comprehensive care to diverse patient populations across the Infectious Diseases Program, including the following services:

Core NP Responsibilities

In addition to population-specific clinical care, the Nurse Practitioner will:

·        Provide clinical coordination for complex infectious diseases cases across the continuum of care.

·        Educate patients and families about treatment plans, monitoring requirements, and escalation pathways.

·        Participate in the development, implementation, and evaluation of clinical pathways, protocols, and quality initiatives.

·        Integrate nursing and medical knowledge within a broad scope of advanced nursing practice.

·        Identify and collaborate with appropriate hospital and community services and resources.

·        Foster interprofessional collaboration in coordinating plans of care.

·        Provide mentorship and coaching to support the development of nursing and interprofessional colleagues.

·        Identify, integrate, and evaluate research-based innovations that improve patient care.

·        Create, apply, and disseminate new knowledge through formal and informal channels.

·        Provide health education tailored to each family's needs, learning preferences, and developmental stage.

·        Lead planning for the successful transition of youth and families to adult programs outside SickKids.

Population-Specific Clinical Care

Children's Rapid Access Clinics

·        Assessment and follow-up of children referred from the Emergency Department or the community requiring rapid assessment of travel-related illnesses (Children's Rapid Assessment of Febrile Travellers' Clinic (CRAFT)) and non-travel related illnesses (Rapid Access Clinic).

·        Perform initial patient assessments, including detailed symptom review, illness chronology, and relevant past medical history.

·        For patients returning from travel, elicit a structured travel and migration history, including destinations, dates, purpose of travel, accommodations, exposures, sick contacts, animal or insect exposures, freshwater exposure, and pre-travel preparation.

·        Working with an Infectious Diseases physician, identify children who require urgent escalation in care, hospital admission, or close outpatient follow-up.

·        Provide counselling about the likely diagnosis, expected clinical course, return precautions, and when to seek urgent reassessment.

·        Support efficient clinic workflow, referral triage, communication with administrative staff, and timely patient disposition.

·        Participate in review of clinic processes, including referral pathways, turnaround times, follow-up needs, and opportunities to improve patient flow.

Infectious Diseases Community Advice Service

·        Coordinate and triage Infectious Diseases consultation requests from community physicians and referring hospitals across Ontario.

·        Serve as the primary liaison between referring providers, Infectious Diseases physicians, patients, and families.

·        Arrange investigations, follow-up care, referrals, and ongoing management plans.

·        Facilitate timely patient flow and transitions across community, ambulatory, and inpatient settings.

·        Support service development, quality improvement, and integration with organizational referral pathways.

Outpatient Parenteral Antimicrobial Therapy (OPAT)

·        Coordinate care for children receiving outpatient intravenous antimicrobial therapy.

·        Monitor treatment safety and effectiveness, including laboratory investigations and therapeutic drug monitoring.

·        Lead coordination of outpatient vancomycin therapy, including serum drug level and renal function monitoring.

·        Collaborate with physicians, pharmacists, home care providers, patients, and families to ensure safe therapy delivery.

·        Promote antimicrobial stewardship and continuity of care through ongoing assessment, education, and follow-up.

·        Support alternatives to hospitalization by enabling safe outpatient management of complex infections.

Special Immunization Clinic

·        Provide initial consultation and follow-up for infants exposed to biologics in utero who require rotavirus counselling.

·        Provide childhood vaccine counselling for immunocompromised children.

·        Provide childhood vaccine counselling for children with complex underlying medical conditions.

·        Provide catch-up immunization counselling for children who are unimmunized or partially immunized.

Here's What You'll Need

·        Master of Nursing or equivalent degree with a clinical focus.

·        Nurse practitioner certificate or equivalent.

·        Current registration with the College of Nurses of Ontario in the Registered Nurse (Extended Class).

·        Three to five years of clinical nursing experience; Nurse Practitioner experience is an asset.

·        Demonstrated leadership ability.

·        Ability to act as a change agent and foster teamwork and consensus-building.

·        Innovative critical thinking, excellent communication skills, and strong problem-solving ability.

Employment type: Full-time, 37.5 hours per week (1.0 FTE)

Term: One-year contract
