INTAKE COORDINATOR

We are seeking an Intake Coordinator to serve as the first point of contact for children, youth and families seeking rehabilitation and developmental services. The Intake Coordinator will be responsible for coordinating the intake and referral process, responding to inquiries, guiding families to appropriate programs and services, and supporting timely access to care. Working collaboratively with an interdisciplinary team, this role will help ensure a seamless, child and family-centred experience while contributing to the delivery of high-quality paediatric rehabilitation services.

Boomerang Health, powered by SickKids, is a multidisciplinary paediatric clinic that is independent but wholly owned by The Hospital for Sick Children (SickKids). We offer physician, rehabilitation and developmental services for children and youth up to 18 years of age.

SickKids opened Boomerang Health in April 2013, an 11,000-square-foot healthcare centre in Vaughan, Ontario, to provide York Region and surrounding communities with greater access to community-based services for children and adolescents. JOB SUMMARY

  • Serve as the first point of contact for rehabilitation and developmental service inquiries by telephone, email and in person
  • Communicate with families, the general public and referring sources to respond to inquiries and provide information regarding services
  • Independently assess service needs and make recommendations regarding appropriate programs and services
  • Manage, organize and track incoming referrals and intake communications
  • Review referrals and assess appropriateness for service using established criteria and consultation with clinical team members
  • Create and maintain confidential patient records in the electronic medical record (EMR) system, including consent forms, test results and supporting documentation
  • Obtain and coordinate required demographic and health information in accordance with privacy legislation and organizational policies
  • Manage intake inquiries, appointment scheduling and service waitlists while ensuring referrals and inquiries are followed through to completion
  • Keep families informed of anticipated wait times and next steps in the referral process
  • Support continuous process improvements and recommend enhancements to systems and workflows
  • Perform data entry and maintain referral tracking systems
  • Complete small projects and assignments as requested
  • Redirect unsuitable referrals to appropriate agencies, hospitals or community services and assist families in identifying alternative resources
  • Prepare monthly statistical reports and annual summaries
  • Provide front desk reception coverage as required
  • Perform other duties as assigned

QUALIFICATIONS

  • Postsecondary degree or diploma in Psychology, Sociology, Child and Youth Care, Health Sciences, Rehabilitation, Health Administration or a related field
  • Minimum of three to five years of experience in intake, care coordination, client navigation or healthcare administration, preferably within paediatric rehabilitation, developmental services, children's treatment centres or community healthcare settings
  • Accounting knowledge and experience is considered an asset
  • Experience using an electronic medical record (EMR) system

KNOWLEDGE, SKILLS AND ABILITIES

  • Strong administrative and organizational skills with the ability to manage multiple priorities, work under pressure and meet deadlines
  • Demonstrated initiative and strong problem-solving and critical thinking skills
  • Excellent time management skills and attention to detail
  • Ability to handle complex and sensitive matters with professionalism, diplomacy and confidentiality
  • Exceptional interpersonal skills and the ability to work effectively with children and families
  • Strong collaboration and teamwork skills
  • Excellent verbal and written communication skills, including grammar and proofreading abilities
  • Proficiency with Microsoft Office applications, including Word, Excel, PowerPoint and Outlook
  • Knowledge of medical terminology is considered an asset
