Our A&P business is at the forefront of the shift to value-based care, tackling challenges head-on and delivering innovative solutions that bridge previously siloed care environments. By expanding our real-time care collaboration network, we empower Health Plans and ACOs to improve transitional care management outcomes across care settings.
