Posting Description
We are recruiting for a Health Transition Specialist to join the Haematology/Oncology team to work with children and adolescents with hemoglobinopathies. The Haemoglobinopathies Transition Program works clinically with the Haematology team in their efforts to support children and adolescents with chronic conditions to effectively transition from childhood to adulthood and from the child to the adult health care system.
This role requires a strong understanding of, and commitment to, equity'informed and culturally responsive practice. The successful candidate will demonstrate knowledge of health equity, the impacts of social determinants of health, and the ways systemic and anti'Black racism influence individual, family, and community well'being. Candidates should be committed to anti'oppressive practice, working collaboratively with communities, and advancing equitable outcomes

Here's What You'll Get to Do:
- Provide and promote excellence in transition patient care, patient and staff education, consultation,'quality improvement initiatives'and research.
- Partner with professionals from both pediatric and adult hemoglobinopathy programs at Sick Kids and Toronto General Hospital.
- Play an important role in representing and promoting the program's goals, objectives, activities and research initiatives.
- Act as a resource to colleagues and participate in integrating research into practice both within SickKids and in the community.
- Involved with creating, utilizing and disseminating new knowledge.
Here's What You'll Need:
Essential:
- You have a Master's degree in Social Work, or are a Registered Nurse or you are an Occupational Therapist.
- Registration with the appropriate professional college is required.
- Experience in adolescent and young adult health (including chronic conditions and high-risk behaviors), as well as the area of health care transitions.
- Demonstrated ability to actively lead and promote program development.
- Ability to act in both autonomous and team roles.
- Excellent communication and public speaking skills.
- Knowledge of both theoretical and practical approaches to healthcare transitions.
- Experience with care coordination in a hospital setting.
Assets:
- Knowledge and experience navigating care within the paediatric and adult health care system preferred.
- Understanding and commitment to EDI informed practices, health equity, and the influences of social determinants of health.
- Strong ability to build relationships and collaborate across sectors (i.e. healthcare, workplace, academic, developmental).
- Experience in research and quality improvement in a healthcare setting.

- 1.0 FTE; 37.5 hrs/week
