This opportunity is with our Good Shepherd - Women's Services - Community Support Services
How You Will Make an Impact
Reporting to the Program Manager of Community Support Services, the successful candidate will be responsible for the provision of service to clients of the Second Stage and Transitional Housing programs. The candidate will assist households in the development of housing-based personalized care plans. As part of your role, you will maintain a caseload and regularly engage with clients to monitor progress and provide support. Additional responsibilities include:
- Intake, safety assessment and planning, and assisting in the development of individualized care plans.
- Housing-based functions including housing searches, viewings, applications, lease signings, advocacy and referrals.
- Discharge planning
- Plan and participate in community events, and public education opportunities
- Plan and facilitate groups/actives
- Maintain confidentiality with regards to clients/staff and Centre's records
- Ensure adherence to occupational health and safety regulations, corporate and program policy
What You'll Bring to Our Team
- Post-secondary education in social services or a related field is required
- Have strong knowledge of, and experience working with a range of issues impacting women, gender diverse individuals and families including homelessness, gender-based violence, immigration, poverty, mental health and substance use
- Minimum of two-years' experience providing housing-based supports to individuals accessing the homelessness serving sector.
- Knowledge of (or willingness to learn) about the housing system in Hamilton, Residential Tenancies Act, and the Ontario Human Rights Code
- Understand and be comfortable working from a person-centered, harm reduction, trauma-informed approach
- Fluency in a 2nd language is an asset
- CPR certification or willingness to be certified is required
- Valid G license and ability to drive company vehicle required
- Excellent interpersonal and conflict resolution skills
