In this role, you will:
- Provide support to seniors during hospital discharge process
- Support clients during the transition from hospital to client’s home in accordance with established service plan
- Complete basic errands for clients as required, in accordance with established service plan (i.e. grocery pick up, etc.)
- Complete basic home management duties, in accordance with established service plan.
- Complete risk analysis of home using standardized Risk Assessment tool
- Facilitate client and caregiver engagement., i.e. providing applicable information on client related issues
- Facilitate referrals to Community Support Services and discharge process as per program guidelines
- Handle and stores hazardous products safely (WHMIS)
- Maintain comfort and safety of client’s immediate environment
- Identify and reports any unsafe or hazardous equipment and situations to the Program Coordinator
- Take measures to ensure personal safety
- Take measures to protect the health and safety of clients
- Observe and report any concerns related to the client or their environment to the Program Coordinator
- Perform other related duties as assigned and required; in consultation with the Program Coordinator
- Provide companionship to clients being transitioned home
- Maintenance of confidentiality of clients
What we are looking for:
- Must be between 15 and 30 years of age (inclusive) at the start of employment.
- Excellent interpersonal and recording skills.
- A caring, non-judgemental individual.
- A self-motivated individual who takes initiative.
- Proficient in troubleshooting and resolving problems.
- Fluency in English is required, French is an asset.
- Good writing and verbal communication skills.
- Standard computer knowledge (Microsoft Office and Internet Research).
