Employment Status: Temporary Full-time (2 Months- Summer Student)

In this role you will:

  • Provide support to seniors during hospital discharge. 

  • Support clients during the transition from hospital to home in accordance with established service plan. 

  • Complete basic errands for service participants as required. 

  • Conduct basic home management duties. 

  • Complete risk analysis of home using standardized Risk Assessment tools.

  •  Facilitate service participant and caregiver engagement. 

  • Facilitate referrals to Community Support Services. 

  • Handle and store hazardous products safely. 

  • Maintain comfort and safely of service participants immediate environment. 

  • Identify and report any unsafe or hazardous equipment or situations to the Program Manager. 

  • Take measures to ensure personal safety and to protect the health and safety of service participants. 

  • Observe and report any concerns related to the service participant and or their environment to the Program Manager. 

  • Provide companionship to service participants being transitioned home. 

  • Assist Program Manager with statistics and reports as needed.


What we are looking for:

  • Excellent interpersonal skills and the ability to accurately document and record information.

  • Strong computer skills, particularly in Microsoft Office applications including Word, Excel, and Outlook.

  • Proven organizational abilities with a keen eye for detail and the capacity to manage and interpret complex information.

  • Outstanding written and verbal communication skills.

  • A proactive mindset with the ability to take initiative and work independently.

  • Solid problem-solving skills and the ability to troubleshoot and resolve issues effectively.


Working conditions:

  • Be between 16 and 30 years of age at the start of the employment.

  • Have a valid Social Insurance Number.

  • Valid Driver's license
